We are contracted with several different insurance companies, which may change from time to time. To find out if your insurance plan is accepted, please call your clinic or our Patient Accounts Department at (360) 318-9705 or (866) 494-4414. Please check with us before changing insurance plans.
NEW PATIENTS: Please use our Find a Provider tool to identify providers who are accepting new patients, and the type of insurance each provider currently accepts.
We will bill your insurance company first*; any remaining balance will be billed to you. We have contracts with the following commercial plans for 2024:
- Aetna
- Blue Cross/Blue Shield
- Cigna
- First Choice
- Healthcare Management Administrators
- Kaiser Permanente
- Molina
- Premera
- Regence
- USFHP (a TRICARE option)

If you don’t see your plan listed, please call for more information: (360) 318-8800 or (866) 494-4414. We also offer private pay options for patients with no insurance.
*Starting January 1, 2024, we will no longer see patients on health insurance plans that are not contracted with Family Care Network. This change applies to all non-contracted plans that serve as a patient’s primary coverage. Examples include United Healthcare; military plans from Tricare, Healthnet, and VA/ChampVA; Apple Health plans from Amerigroup, CHPW, and United Healthcare; and any other non-contracted plans.
