IMPORTANT UPDATE:

On June 3, Family Care Network changed to a new patient portal, called MyChart.

Go to mychart.fcn.net or download the app to create an account. If you need help or have questions about your account, please call your clinic. 

Please note: It will take time to migrate patient information from our current portal to MyChart. This means some patients may see their information in MyChart sooner than others. Over time, patients will see past and current activity in MyChart.

InTouch patent portal accounts will remain accessible to review past medical information. Appointment scheduling and messaging is disabled.

Important FAQs

Can I use my InTouch username and password to log in to MyChart?

No. If you are accessing MyChart for the first time, you need to set up an account as a New User. You will be asked to set up a username and password for MyChart during this process.

What is the age requirement for a Family Care Network MyChart account?

FCN patients must be age 13 or older to have a MyChart account.

  • Adults age 18+ can self sign-up online or through the MyChart app, with or without an activation code.
  • Patients age 13-17 require an activation code to create an account (online or app). Please call your clinic for more information.

Can a parent access their child’s (age 12 and under) medical information in MyChart?

Yes. Once the parent has set up a MyChart account for themselves, they can use the “Request Access to a Child” form available in the main menu, under “Sharing”. They can also call the clinic to request access, or request access in person at the clinic.

Can a parent create or access a MyChart account for a teen (age 13-17)?

No. Teen patients can set up their own account with an activation code provided by their clinic, either by calling or in person.

How do I get help with a MyChart account?

Please call your clinic during business hours. They can help with log in issues, password resets, messaging issues, etc.

You can also request a direct activation link by calling your clinic, which makes creating a MyChart account  easier.
