Medical Records

Family Care Network can provide a copy of your medical records to you or another provider upon request. Please choose from the following options to submit your request:

1. Go to the Sharing Hub in your Family Care Network MyChart account, where you have options to submit a records request, download a copy of your records, set up proxy access for a friend or family member, or grant one-time access to your record with Share Everywhere.    • If you don’t have a Family Care Network MyChart account, please go to mychart.fcn.net or download the app to get started.
2. Call the clinic where you were seen to make a request. The clinic can provide more information on what steps you’ll need to take, based on what records you need. You may need to complete one of these forms:
   • Incoming Request for Medical Records
   • Outgoing Request for Medical Records
3. Visit the clinic in person to fill out a medical records request form, or request a form at your next visit.
