Family Care Network is pleased to announce the addition of its newest location — Chuckanut Family Medicine!
Located in the heart of Fairhaven, Chuckanut Family Medicine is an independent, physician-owned practice started by Jamie Kendall-Weed, MD, in 2019. Emily Holden, ARNP, joined the team in 2021.
The decision to bring Chuckanut Family Medicine into the Family Care Network organization was made by FCN’s board of directors. They agreed that adding CFM as a new FCN location reinforces the company’s mission to build a positive and rewarding healthcare experience for patients in the communities they serve.
Family Care Network and Chuckanut Family Medicine share a common set of values that puts patients first. We are committed to providing high quality patient care that is based on strong patient relationships. This is an exciting opportunity to bring a shared vision for excellent, independent, patient-centered care to a larger population.
By joining with Family Care Network, Chuckanut Family Medicine will benefit from centralized administrative support and will offer patients a new, convenient on-site lab draw station.
CFM will also welcome Dr. Matt Werner of Family Care Network’s Squalicum Family Medicine, who will move his practice to Chuckanut Family Medicine in July. He is a well-respected and trusted physician who has been with Family Care Network for 18 years.
Chuckanut Family Medicine will officially become part of FCN on July 1. The first day seeing patients in the office as Family Care Network will be Wednesday, July 6, to allow time for the transition and training of providers and staff. Patients will have access to an on call provider over the Independence Day holiday weekend and during training days (July 1-5).
